Location: Panama City, Florida
Compensation: $109,762 - $164,570 (based on experience; rarely approves max)

This leader will oversee care coordination, discharge planning, utilization review, and social work functions while ensuring high-quality patient outcomes and operational excellence. Candidates must bring strong acute hospital case management leadership experience and be comfortable directing a complex, high-volume environment.
Position Overview
The Director of Case Management provides strategic direction, operational oversight, and performance leadership for all case management and social work services. This includes evaluating departmental processes, optimizing staffing and resources, ensuring compliance with regulatory and organizational policies, and partnering closely with clinical and administrative leadership to advance patient throughput and quality outcomes.
Key Responsibilities
• Lead and evaluate all case management operations, including model design, staffing, onboarding, competencies, and performance
• Manage FTE allocation, skill mix, and productivity to meet departmental goals
• Ensure proper execution of care coordination and discharge planning workflows, with consistent documentation and timeliness
• Oversee core case management functions including utilization review, transitions of care, and interdisciplinary collaboration
• Partner with the CFO and Division CM Leader to achieve financial and operational targets
• Use data analysis, KPIs, and process improvement strategies to drive sustained performance improvements
• Ensure full compliance with policies, procedures, and regulatory requirements
• Build strong relationships with CMO, CNO, ancillary leaders, compliance, and legal teams
• Provide leadership, coaching, and accountability for both direct and indirect reports
• Uphold organizational mission, values, and code of conduct
Required Qualifications
• 3+ years of recent acute hospital case management experience (within the last year)
• 2+ years of case management leadership
• Bachelor's degree required
• Master's degree (Nursing, Health Administration, Business Administration) preferred
• RN license or LCSW required
• Hospital discharge planning experience required
• CCM or ACM preferred
