Director of Case Management

Leading Hospital in the Hudson Valley is seeking and RN or LMSW to lead it's Case Management Department!

• Lead utilization review, discharge planning, and care coordination programs.
• Manage department budgets, supervise staff, ensure regulatory compliance, and work with medical teams to optimize patient lengths of stay, transitions of care, and resource use
• Hire, train, schedule, and evaluate case managers, supervisors, and support personnel.
• Maintain alignment with HIPAA, Joint Commission, and state/federal health guidelines; act as a primary liaison to payers and medical staff.

Background:

RN or LMSW in the State of NY

5+ years in leadership in Case Management in an Acute Care Setting

Understanding of assessment, planning, and managing patient care.

Familiarity with all state and regulatory requirements related to case management and social work.
