Job Details
- Provide leadership and oversight for the Case Management department, including 11 direct reports (RNs, LPN, MSWs, LSW, and other allied health professionals)
- Guide safe, timely, and effective discharge planning across the hospital
- Support Utilization Review processes in coordination with the case management team
- Monitor and manage case manager caseloads (typical caseload of 13 patients per CM); step in to carry a patient assignment when census is low or staffing is short
- Maintain working knowledge of case managers' patient assignments and actively support the team as needed
- Foster strong interdisciplinary communication across nursing, therapy, and hospital leadership
- Partner with the RDCM on training, orientation, and ongoing education initiatives
- Participate in hospital leadership meetings and contribute to department strategy
