Director of Case Management – Inpatient Rehab (Relocation)

Jobot is seeking a hands‑on Director to lead and rebuild Case Management at a 40‑bed inpatient rehab hospital in Sioux Falls. You will direct utilization review, care coordination, and discharge planning, while growing a team of CM staff and contractors and driving a culture focused on discharge outcomes.

The role requires an RN with BSN and active license, 3+ years hospital CM experience, CCM/ACM or enrollment within the first year, and a track record of improving patient discharge efficiency.
