Responsibilities

• You will be working with your teams to address the patient’s transitions between the hospital and Home Care, Long Term Acute Care and Skilled Nursing Facilities.

Qualifications

• The qualified candidate should have three years clinical experience in case management.
• Two-Five years leadership experience with focus on case and disease management in an acute care hospital setting.
• Education Requirement: Bachelor’s Degree from an accredited school, college or university as a Registered Nurse or Social Worker. Master’s degree is a plus.
• MINIMUM EXPERIENCE: 2 years in acute hospital case management and leadership position
• Our client uses McKesson InterQual®. Business Planning experience required.
• It is preferred you hold Certification in Case Management

Competitive six figure salary plus 12% performance bonus
