ACMH in Manhattan seeks a Director of Intake & Transition Services to coordinate agency intake and promote client transitions to more independent living. Reports to the Senior Director for Residential Programs.
Schedule is Monday–Friday, 9:00am–5:00pm. Responsibilities include reviewing applications, screenings, admissions referrals, waitlists and census reports while liaising with hospitals, clinics, shelters and housing providers to ensure policy-compliant admissions and transitions.
