Job Summary

The Director of Case Management will oversee case management activities at Community Hospital North and Community Heart and Vascular Hospital. The role provides onsite leadership to ensure daily department operations function effectively and efficiently to meet the needs of all customers and regulatory bodies. The Director consults, directs staff, and facilitates compliance with departmental processes, policies, and regulatory agencies. Additionally, the Director leads continuous improvement initiatives, evaluates operational issues, and makes recommendations to promote cost‑effective, quality care.

Responsibilities

  • Provide onsite leadership and support to staff and customers to enable effective and efficient department operations.
  • Offer consultation and direction to staff to facilitate compliance with departmental processes, policies, and regulatory agencies.
  • Guide employees and customers and facilitate continuous change and improvement.
  • Evaluate and monitor operational issues and recommend improvements to promote cost‑effective, quality care.
  • Collaborate with interdisciplinary teams to effect change and support growth and development of team members.

Qualifications

  • Graduate of an accredited School of Nursing and/or Social Work.
  • If a Registered Nurse, a Bachelor’s Degree is required with a Master’s Degree required within five (5) years of assuming the role.
  • If not a Registered Nurse, a Master of Social Work (MSW) is required.
  • Licensed as a Registered Nurse (RN) with a valid license to practice in Indiana, or a current LSW or LCSW licensed by the State of Indiana.
  • Three (3) to Five (5) years of clinical experience in a hospital setting as an RN or MSW.
  • Minimum of two (2) years of management or leadership experience.
  • Two (2) to Three (3) years of case management experience.
  • Demonstrated understanding of quality improvement processes at both the unit and network level.
  • Knowledge of the relationship among individual patients, units, and departments to facilitate achievement of optimal goals within those relationships.
