POSITION SUMMARY

ENL Case Management Exempt Compensation range: $55.23 - $89.68. Your rate of pay will be based on applicable experience.

Shift: Days. Shift length: 8 hours. Days off: Saturday & Sunday. Hours per pay period: 80. Sign‑on bonus: $10,000.00.

The Director is accountable for operations and the integration of all care coordination strategies for Enloe Health. The Director coordinates the design, development, implementation, and monitoring of the organization’s discharge planning functions and Care Coordination, integrating services to hardwire safe, seamless transitions for patients across the continuum. The Director guides staff and managers in case management and social work toward achieving clinical, financial, quality, and utilization goals through effective management, communication, and role modeling.

The Director participates in program development and performance improvement, consistently demonstrates the core values of Enloe Health, and works closely with physicians and all departments to guide operational direction, planning, and coordination of services. The Director participates in the development and management of department budgets and productivity targets, manages human resources utilization, promotes employee satisfaction, supports staff development, and utilizes or supports the progressive discipline process when appropriate.

Other responsibilities include leading employees, supporting organizational goals and decisions, implementing necessary changes, participating in strategic planning for service needs and resource utilization, maintaining and promoting quality service and best practice, participating in quality improvement processes and ensuring regulatory standards are met, and ensuring services are provided with consideration of age‑specific physiological, emotional, and cognitive needs. The Director maintains communication with V.P. and administration regarding level of care/service and satisfaction, translates professional standards into policies and procedures, researches interdepartmental problems, and promotes respectful, responsive communication between departments.

Additional duties include providing leadership and mentorship to case management managers and leads, ensuring adherence to standards of care, acting as a resource to other departments, implementing and monitoring care coordination services, establishing, evaluating, and monitoring case management processes and policies, coordinating compliance with California Medical Review, Inc. (CMRI), serving as an internal resource and consultant to management and medical staff, and communicating clinical outliers impacting financial and patient outcomes in collaboration with physicians and external agencies.

QUALIFICATIONS

  • Minimum: Five years of hospital clinical experience.
  • Master’s degree in nursing OR Master’s degree in health care administration and Bachelor’s degree in nursing.
  • Desired: Master’s Degree in Business Administration.

LICENSES & CERTIFICATIONS

  • Minimum: Current licensure with the California Board of Registered Nursing.
  • Current CPR recognition.
  • Accredited Case Manager, Certified Case Manager (ACM or CCM) within 1 year.

SKILLS / KNOWLEDGE / ABILITIES

Knowledge of case management, care coordination, and utilization review processes; knowledge of regulatory requirements and quality improvement processes; ability to learn financial management, reporting, analytics, quality improvement, and human resources management; ability to monitor, evaluate, and administer resources of Care Management and Social Work; strong written and verbal communication; leadership, motivation, delegation, analysis, problem solving, critical thinking, and analytical skills; expertise in human resource management and staff development; ability to build positive relationships with physicians and external agencies; excellent understanding of reimbursement; strategic planning and operational work plan skills; confidentiality, integrity, creativity, and initiative.
