Director of Care Coordination (RN).

This highly visible leadership opportunity includes system-wide oversight of:

• Case Management and Utilization Review
• Discharge planning and length-of-stay performance
• Palliative Care coordination and Social Work services
• Approximately 65 team members across two hospital campuses
• Two site-based Case Management Managers and a Utilization Review Manager

The organization is seeking a collaborative, data-driven leader who can manage a large department, strengthen physician partnerships and improve patient progression across the continuum of care.

Requirements

• RN required
• Case Management or Care Coordination leadership experience within a large, complex hospital or health system
• Five to seven years of acute care hospital experience
• At least three years of leadership experience within Case Management or Care Coordination
• Master’s degree required (if not in Nursing, must hold a BSN)
• CCM certification strongly preferred
