Position Overview

The Day Habilitation Specialist provides individuals with developmental disabilities with skill development through a combination of community and classroom learning experiences at our program located on 5th Avenue in Brooklyn, NY.

Pay Rate: $19.00 per hour. Flexible scheduling, union benefits, and a 10% internal promotion rate organization-wide.

Responsibilities

• Assist each consumer in skill development in areas such as independent living, adaptive behaviors, socialization, recreation, and activities of daily living.
• Supervise consumers in the community, using various experiences to reinforce skills being taught in group sessions while ensuring their safety.
• Lead group sessions with consumers and facilitate discussions to provide learning skills to be used in their community.
• Provide annual and semi-annual progress reports regarding valued outcomes.
• Assess the needs of lower functioning and/or non-verbal consumers through observations and evaluations; develop and address their valued outcomes.
• Provide input and ensure compliance with the valued outcomes in the ISP and Day Habilitation Plan.
• Provide weekly documentation of outcomes being worked toward with each individual in accordance with their Day Habilitation Plan.

Qualifications

• High School Diploma or GED required; Associate’s Degree preferred.
• At least one year of experience providing direct care services to adults with a developmental disability.
• AMAP certified, SCIP‑R trained, CPR & First Aid trained.
