Description:
The Dialysis Registered Nurse provides direct and indirect nursing care to patients receiving dialysis therapies.
This position functions in both inpatient and outpatient settings and is responsible for the assessment, planning, implementation, coordination, and evaluation of dialysis care.
Responsibilities include dialysis treatment administration, patient and caregiver education, and care coordination.
The RN collaborates closely with interdisciplinary team members to ensure safe and high-quality dialysis care.
Participates in performance improvement activities and supports the mission and values of the organization.
Performs comprehensive nursing assessments for patients receiving dialysis therapies.
Initiates, monitors, and discontinues dialysis therapies as ordered.
Assesses patients' responses to dialysis therapies and communicates concerns to the interdisciplinary team.
Monitors patients during dialysis treatments and responds appropriately to complications or changes in patient condition.
Administers medications, blood products, and intravenous therapies as per orders.
Maintains accurate documentation within the electronic medical record.
Provides education and training to patients and caregivers receiving home dialysis.
Coordinates routine clinic visits for home dialysis patients.
Assesses catheter exit sites, provides education, and communicates concerns.
Coordinates laboratory reviews and other required home dialysis assessments.
Provides telephone triage and clinical support for home dialysis patients.
Coordinates transitions of care to promote continuity.
Ensures dialysis equipment is functioning appropriately before treatment initiation.
Performs required safety checks and monitors water quality.
Cleans and disinfects dialysis machines and treatment areas according to standards.
Participates in infection prevention and control practices.
Collaborates with interdisciplinary team members regarding patient care planning.
Provides education and clinical support to patients and team members regarding dialysis therapies.
Responds to dialysis-related emergencies as appropriate.
Participates in rotating on-call coverage for urgent clinical support.
Participates in quality assessment and performance improvement activities.
Supports program development and continuous process improvement initiatives.
Reports unusual incidents, patient concerns, and equipment issues according to policy.
Participates in required organizational education applicable to the position.
Performs other related duties as assigned.
Ability to communicate effectively with patients, families, and other healthcare team members.
Strong critical thinking skills are essential for this role.
Physical and sensory functions required include auditory ability for patient alarms, clear communication, and visual acuity for clinical tasks.
Motor coordination is required for using medical equipment and performing patient care.
The position involves occasional lifting and physical activities consistent with patient care.
Working conditions are primarily indoors in a healthcare environment, subject to variable temperatures and exposure to infectious diseases.
The healthcare environment can be fast-paced and unpredictable, requiring quick responses to changing circumstances.
