Providing remote clinical management, the full-time Delaware Licensed Virtual Swingternist will assess patient health, develop individualized care plans, and collaborate with a hospitalist physician while working from home. Key responsibilities Perform cross coverage on patients admitted under inpatient status and assess their health through complete medical history and chart review Analyze patient data to formulate individualized care plans in collaboration with the multidisciplinary team and monitor their effectiveness Complete timely documentation in the electronic health record (EHR) and ensure efficient patient flow through the system Required qualifications Acute Care Nurse Practitioner or Physician Assistant license in Delaware and Maryland Prescriptive Authority and DEA registration in Delaware and Maryland ACLS certification required
