Aspirus Wisconsin Rapids Home Care is seeking a Registered Nurse - Case Manager to join our Wisconsin Rapids Home Health team. This role oversees assessment, planning, implementation, coordination and evaluation of nursing care for Home Health patients and families, across ages and settings.
Full-Time or 1.0 FTE with day shifts and on-call 1-2 times per week. Requires Wisconsin (or Michigan) RN license, CPR/BLS, and ability to travel to patient homes; driver’s license and insurance required.
