Delivering high-quality nursing care remotely, the full-time Remote Care Nurse II will conduct patient assessments, develop care plans, and coordinate with healthcare teams to optimize patient outcomes through telehealth technologies. Key responsibilities: Conduct thorough remote patient assessments via phone or video conferencing to evaluate health status and identify care needs Develop, implement, and update individualized care plans in collaboration with patients and healthcare teams Monitor patient progress and adherence to treatment plans, providing education and guidance to promote self-care and disease management Required qualifications: Current and unrestricted Registered Nurse (RN) license in the United States Minimum of two years of clinical nursing experience, preferably in case management, chronic disease management, or telehealth Proficiency with electronic health record (EHR) systems and telehealth technology platforms Ability to work independently in a remote environment while managing multiple patient cases efficiently Strong communication skills to effectively engage with patients and interdisciplinary teams remotely
