VitalCaring is seeking a Chaplain / Spiritual Counselor to provide compassionate spiritual and emotional support to home health and hospice patients and their families. In this role, you will complete spiritual assessments, offer counseling and bereavement support, and collaborate with an interdisciplinary care team to honor each patient’s beliefs, culture, and preferences. You will visit patients in their homes, lead prayers or rituals as requested, and help families navigate serious illness and end-of-life with dignity.
