To support outpatient parenteral antibiotic therapy, the per diem Connecticut Licensed Clinical Pharmacist will resolve drug distribution issues, interpret clinical and pharmacokinetic lab data, and provide drug information to healthcare professionals while working remotely as needed. Key responsibilities Resolves drug distribution problems to ensure accurate and timely medication delivery Interprets clinical and pharmacokinetic lab data to formulate medication monitoring plans Documents medication use variances and adverse drug reactions, providing solutions to prevent recurrence Required qualifications Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree Two years of experience in an acute care hospital pharmacy is preferred Licensed to practice pharmacy in Connecticut or eligible for reciprocation Knowledge of pharmacology and therapeutics, along with pharmacy systems of drug distribution Demonstrated ability to work collaboratively with healthcare professionals
