Escambia County Healthy Start Coalition seeks an ambitious and experienced part time Connect Program Navigator. The successful candidate will possess a 4-year college degree and non-profit experience in human services, public health, social work, psychology or a closely related field. Must have demonstrated superior oral and written communication skills, organization skills, and public speaking acumen. Duties include engagement of community partners, relationship building, and enrollment of Healthy Start clients in services in a case management platform, while providing support and resources to pregnant women, new mothers, and infants.

The Florida Legislature created the Healthy Start program to establish, implement, and monitor local Healthy Start care coordination systems that provide women with access to prenatal care and wraparound services that promote healthy pregnancies, improve birth outcomes, and provide children from birth to age three with access to services that promote normal growth and development.

The primary objectives of the Healthy Start Initiative are to reduce infant mortality and morbidity, improve pregnancy outcomes, and enhance the health and development of children from birth to age three.

Duties and Responsibilities

Community Partnerships & Outreach

  • Work with the Community Liasion to develop and maintain relationships with healthcare providers, community organizations, social service agencies, and other referral partners.
  • Represent Healthy Start at community meetings, health fairs, outreach events, and coalition activities.
  • Conduct presentations to community partners regarding Healthy Start services and referral processes.
  • Identify opportunities to increase awareness of Healthy Start programs throughout Escambia County.
  • Serve as an ambassador for the organization within the community.

Client Engagement & Referrals

  • Work in the field at local hospitals and healthcare centers to educate pregnant women and families about Healthy Start services.
  • Assist eligible families with enrollment into Healthy Start programs.
  • Complete client intake and documentation within the designated case management platform.
  • Maintain timely, accurate documentation.
  • Coordinate referrals to internal programs and community resources.
  • Provide culturally responsive customer service and support throughout the enrollment process.

Data & Reporting

  • Track outreach activities and referral outcomes.
  • Maintain accurate records of presentations, meetings, and community contacts.
  • Assist with reporting community engagement activities and performance measures.

Other Responsibilities

  • Collaborate with program staff to improve outreach strategies.
  • Participate in staff meetings and training.
  • Perform other duties as assigned.

What Success Looks Like in This Role

Within the first year, the Navigator will:

  • Establish a presence in the field with key referral partners throughout Escambia County.
  • Increase awareness of Healthy Start services among healthcare providers and community organizations.
  • Consistently represent Healthy Start at community events and outreach opportunities.
  • Successfully enrolling eligible families into Home Visiting services.
  • Maintain timely and accurate documentation of outreach and client activities.
  • Become a trusted resource for providers, community partners, and families seeking maternal and child health services.
  • Contribute to increased Healthy Start screening and referral activity.

Required Qualifications

  • Bachelor's degree in Public Health, Social Work, Human Services, Health Education, Psychology, Communications, or a related field.
  • One to three years of professional experience in community outreach, health education, case management, public health, nonprofit services, healthcare, or a related field.
  • Excellent written and verbal communication skills.
  • Experience building professional relationships.
  • Public speaking or presentation experience.
  • Strong organizational and time management skills.
  • Proficiency with Microsoft Office.
  • Valid driver's license and reliable transportation.

Preferred Qualifications

  • Experience with maternal and child health.
  • Knowledge of community resources.
  • Experience with care coordination or case management software.
  • Bilingual abilities.
  • Reports directly to the Executive Director.
  • Primarily office-based with regular community engagement responsibilities and time in the field at provider offices.
  • Must be available to work periodic evenings and weekends for fundraising events, community outreach activities, meetings, and organizational functions.
  • Must reliably commute to or plan to relocate to Pensacola, Florida before employment begins.

Physical Requirements

This position primarily operates in an office setting but requires regular attendance at community events, meetings, and fundraising activities. Responsibilities may include standing for extended periods, event setup and breakdown, transporting materials, lifting to 25 pounds, and local travel throughout the service area. Reasonable accommodation will be provided as needed.
