Vitality Hospice & Palliative Care is hiring a Registered Nurse Case Manager (RNCM) to join our compassionate team. The role focuses on coordinating per-patient care in the community, conducting admissions and ongoing nursing assessments, and collaborating with the IDT to develop individualized plans of care.
The position is full-time, with a Monday–Friday schedule, some evenings or weekends, and a salary range of $85,000–$100,000 per year. Travel to patient residences is required.
