Spanish Cove At Home is seeking an RN Hospice Case Manager to join our Home Hospice team in Oklahoma. This part-time role will develop and implement individualized hospice plans, perform skilled nursing assessments, and coordinate care for residents and their families.
Candidates must hold an active Oklahoma RN license, BLS/CPR, and have at least 1 year in med-surg, geriatric, or hospice settings; supervisory experience is preferred.
