Altus Hospice Care is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our dedicated nursing team, delivering compassionate end-of-life care in the community. Monday - Friday, 8 am - 4:30 pm, territory: Kingwood and Humble Area.
You will coordinate services, provide hands-on hospice nursing, and guide patients and families through the hospice journey with a focus on dignity, comfort, and clinical excellence.
