Concho Hearts Hospice is seeking a compassionate Registered Nurse Case Manager to join our Beaumont team. You will manage a caseload across the Beaumont area, perform comprehensive nursing assessments, and collaborate with physicians and the care team to develop customized plans of care.
You will educate patients and families on disease progression and symptoms, document visits in our EMR, and participate in IDT meetings, ensuring high-quality, person-centered hospice care.
