Frontier Hospice is looking for a compassionate Registered Nurse Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the Norman, Moore, and Midwest City area. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey.
The role requires a current RN license, 1+ year of acute care experience (hospice a plus), EMR proficiency, CPR, and reliable transportation.
