Hospice Plus is seeking an experienced RN Case Manager to lead skilled nursing visits and coordinate end-of-life care in patients' homes or facilities in Texas. You will assess needs, develop and adjust plans of care, and collaborate with an interdisciplinary team to ensure compassionate care.
You will educate patients and families, monitor symptoms, document care, and participate in quality improvement efforts while supporting bereavement education if needed. A Texas RN license is required.
