Gentiva Hospice is seeking a dedicated RN Case Manager to lead patient care in the home and coordinate with an interdisciplinary team to ensure compassionate, individualized plans of care. The nurse will assess patients, document care, and guide families through the hospice journey.
Responsibilities include ongoing assessments, symptom management, education for patients and caregivers, and collaboration with physicians, social workers, chaplains, and others to provide holistic support.
