Gentiva’s New Century Hospice is seeking a Hospice RN / RN Case Manager to join our team in Texas. You will be the primary contact for patients and families, delivering and documenting skilled nursing care in patients’ homes or other settings while coordinating with an interdisciplinary team to create individualized plans of care.
Responsibilities include comprehensive assessments, symptom management, education for families, accurate documentation, and on-call participation.
