To support patient care initiatives, the part-time Remote Compact States Chronic Care Manager will manage a panel of 30-50 patients monthly, ensuring compliance with care plans and completing billable encounters for chronic disease management. Key responsibilities: Conduct monthly outreach including chart reviews, contact attempts, and care coordination to meet billable encounter goals Develop and implement detailed care plans for patients and collaborate with healthcare providers Attend mandatory meetings to discuss care management strategies and updates Required qualifications: Graduation from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.) with a current COMPACT license A minimum of two years of clinical experience in a relevant healthcare setting Hands-on experience with Electronic Medical Records and proficiency in Microsoft Office 365 applications Ability to work in a HIPAA-compliant environment with a reliable high-speed internet connection Strong organizational and time-management skills, with the ability to learn new technology independently
