Key responsibilities Deliver telephonic coaching and referral services to participants managing chronic health conditions Assess participant needs and communicate with healthcare professionals for effective care coordination Document participant activities and coaching sessions in established tracking software Required qualifications Nursing Degree from an accredited college or university Active and unrestricted Compact Registered Nurse License 2+ years of experience in direct participant coaching Intermediate knowledge of Microsoft Office and database software
