To support patients enrolled in chronic care management and remote patient monitoring programs, the full-time Compact LPN Care Navigator will provide education and support while collaborating with care teams, operating remotely from the United States during standard business hours. Key Responsibilities: Complete phone consultations with patients, offering support and education about chronic conditions Educate patients on health management strategies and assist them in achieving goals aligned with their care plans Maintain accurate patient records and document all interactions according to protocol Required Qualifications: Current, valid Multistate/Compact Nursing License (LPN/LVN) in good standing 3+ years of experience in primary care, cardiology, internal medicine, home care, or chronic care management Proficiency in using electronic health records (EHR) and care management software Reliable internet connection and proficiency with virtual communication tools Empathy and a patient-centered approach to care
