To support after-hours hospice care, the part-time Compact Licensed Triage Nurse will assess patient needs telephonically, provide education and recommendations based on care plans, and collaborate with healthcare providers, all while working remotely for 30-32 hours a week. Key responsibilities Assesses physical, environmental, and emotional factors to determine hospice needs Utilizes electronic health records to develop recommendations and provide patient education Collaborates with pharmacies and other agencies for effective patient management and assigns visits to on-call staff Required qualifications Current, unencumbered RN license in the assigned state One year of experience as a registered nurse Current CPR certification Knowledge of the needs of terminally ill patients and their caregivers Willingness to obtain additional RN licenses in other states
