Compact Licensed Nurse Care Navigator

As a Compact Licensed Nurse Care Navigator working remotely in the MST time zone, the candidate will manage patient relationships through wellness calls, triage vital signs, and collaborate with care teams to promote positive health behavior changes and improve patient outcomes. Key responsibilities: Manage physiological markers such as blood pressure, weight, and blood glucose with clinical appropriateness Provide preventive health and disease management education and coaching through monthly wellness calls Lead collaborative wellness calls to define health goals and manage patient messaging and alerts Required qualifications: Unrestricted RN/LPN/LVN license in a compact state 1-3 years of clinical experience, patient management, or disease management desired Experience performing virtual visits with patients and telephonic care management Ability to work independently with minimal direction Spanish speaking is a plus
