To support patients enrolled in chronic care management and remote patient monitoring programs, the full-time Compact Licensed LPN Care Navigator will provide education and assistance to patients, maintain accurate records, and collaborate with care teams, all while working remotely within the United States. Key Responsibilities: Complete phone consultations with patients, providing support and education about their chronic conditions Educate patients on managing their health conditions and assist them in setting and reaching goals aligned with care plans Maintain accurate patient records and document all interactions and care plans according to protocol Required Qualifications: A current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN) 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring Proficiency in using electronic health records (EHR) and care management software Reliable internet connection and HIPAA-compliant work area Empathy and a patient-centered approach to care
