Supporting patients enrolled in chronic care management programs, the full-time Compact Licensed Bilingual LPN will conduct consultations, educate patients on chronic conditions, and maintain accurate documentation while working remotely. Key Responsibilities: Complete phone consultations with patients, providing support and education about their chronic conditions Educate patients on health conditions and empower them to manage their chronic illnesses effectively Maintain accurate patient records, ensuring all interactions and care plans are documented per protocol Required Qualifications: A current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN) 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management Ability to communicate fluently in Spanish Proficiency in using electronic health records (EHR) and care management software Reliable internet connection and proficiency with virtual communication tools (e.g., Zoom, Slack)
