Community Support Services - Case Manager

Cumberland County Guidance Center is seeking a Full-Time Community Support Services Case Manager to join our team in Millville, NJ. If you're passionate about case management, community mental health, behavioral health services, housing support, advocacy, and helping individuals achieve greater independence, this is a rewarding opportunity to make a lasting impact. You'll work directly with individuals living with mental illness in the community, helping them secure housing, access treatment, connect with resources, and develop the skills necessary to live independently.

As a Community Support Services Case Manager, you'll provide advocacy, support, and care coordination for individuals living independently in the community. Responsibilities include: assisting clients in securing and maintaining housing; supporting community integration and independent living; developing Individual Rehabilitation Plans (IRPs); coordinating services with healthcare providers and community agencies; connecting individuals to mental health, medical, vocational, and social services; assisting with transportation, budgeting, and daily living supports; monitoring progress toward recovery goals; and maintaining accurate clinical documentation.

Required qualifications include one of the following: a Bachelor's Degree in a mental health-related field, a Licensed Registered Nurse (RN) with related experience, or a Licensed Practical Nurse (LPN) with at least two years of related experience. Additional requirements include knowledge of community resources and service agencies, strong communication and advocacy skills, a valid New Jersey driver's license with acceptable driving history, and the ability to work a flexible schedule including evenings, weekends, holidays, and on-call rotation.
