To support a growing healthcare practice, the full-time Community Resource Guide will engage patients remotely and in-person, facilitating care navigation, community resource connections, and addressing barriers to care to improve health outcomes. Key responsibilities Building trusting relationships with patients, families, and healthcare providers to facilitate care coordination Engaging high-risk patient populations to address health needs and assist with care plans and follow-ups Identifying and assessing patients' medical, behavioral, and social needs to support whole-person care Required qualifications At least 3 years of experience in patient navigation, community resource navigation, or care coordination Experience working directly with patients with complex medical and social needs Familiarity with EHR systems and documentation platforms Knowledge of medical terminology and primary care workflows Demonstrated ability to work effectively in a remote environment
