What You'll Do

As a Community Inclusion Specialist , you will:

  • Support adults with developmental disabilities in community-based activities and outings
  • Implement Individual Support Plans and person-centered goals
  • Facilitate meaningful recreational, educational, and social activities
  • Promote independence, self-advocacy, and community participation
  • Provide transportation to community activities and appointments
  • Complete required documentation and progress notes
  • Manage outing budgets and activity expenses as needed
  • Provide basic first aid and ensure participant safety

Qualifications

Required

  • High School Diploma or equivalent
  • Minimum three (3) years of experience working with adults with developmental disabilities
  • Valid driver's license & vehicle
  • Basic computer skills
  • Ability to lift up to 50 pounds
  • Strong communication and interpersonal skills

Preferred

  • Bachelor's Degree in Human Services or a related field
  • Experience facilitating community-based programming

Schedule

  • Full-Time
  • Community-Based Position
  • Flexible schedule as needed to support program activities
  • Some evening or weekend hours may be required

Physical Requirements

  • Ability to lift up to 50 pounds
  • Ability to stand, walk, bend, and accompany individuals in community settings
  • Ability to transport participants safely in the community using personal vehicle
  • Ability to perform essential job functions with or without reasonable accommodation
