We have an exciting opportunity for a Community Health Worker (CHW) to join Lend a Hand, part of our growing care management program! Our CHWs provide culturally appropriate complex care management in the primary care setting. We work with each patient for approximately 6 months, helping them to overcome barriers, navigate the healthcare system, and make meaningful progress toward health-related goals. Summary:
The Community Health Worker role assists patients in accessing resources/services, supports disease self-management, and coordinates culturally-appropriate care and services.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: High school diploma or GED preferred.
- License/Certifications: If driving required for role, Valid Driver’s license, clean driving record, and personal vehicle for use required.
- Experience: Two years of experience working in a social service or health care setting or equivalent educational setting preferred.
- Additional Skills/Requirements Required: Must be a resident of the community being served with good knowledge of the resources of their community.
- Additional Skills/Requirements Preferred: Basic computer skills preferred.

The base range is an estimate and does not reflect the full value of our total compensation package.
The pay range for this position is $20.68 to $25.10 per hour.
Compensation for part-time positions is determined on a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
