Community Health Worker

To support adolescents and young adults with childhood-onset chronic conditions, the part-time Community Health Worker will provide peer coaching and guidance remotely, helping families navigate healthcare systems and achieve personal health goals. Key responsibilities Provide peer health and wellness coaching through telephone and virtual platforms, utilizing lived experience to promote engagement and self-management Support patients in navigating healthcare systems and community resources, helping them understand available services and overcome barriers to care Maintain accurate documentation of coaching encounters and patient progress in accordance with program requirements Required qualifications High school diploma or equivalent One year of experience in a position involving patient, family, public, or community contact Knowledge of and experience navigating youth-serving systems related to special healthcare needs Ability to build trusting relationships with patients, families, and healthcare team members Basic computer skills for documenting services and interactions accurately
