What You'll Do

  Connect patients with healthcare and community resources

  Provide outreach, education, and care coordination support

  Advocate for patients and help reduce barriers to care

  Collaborate with medical providers and care teams

  Document interactions and services in the medical record

Minimum Qualifications

n
• n

  Community Health Worker (CHW) Certification required

  n
n
• n

  Strong communication and patient advocacy skills

  n
n
• n

  Knowledge of community resources and social service programs

  n
n
• n

  Passion for serving underserved and vulnerable populations

  n
n

Experience working with homelessness, behavioral health, substance use, housing resources, HIV services, or telehealth outreach is highly valued.

Minimum Requirements:

  Professional License in Behavioral Sciences, Human Services, or Social Services preferred

  Bachelor's Degree in Behavioral Sciences, Human Services, or Social Services fields preferred

  Experience in Case Management, Behavioral Sciences, Human Services or Social Services preferred

  Complies with all relevant professional standards of practice

  Current CPR if applicable

• n

  TB questionnaire, PPD or chest x-ray if applicable

  Current Health certificate (per contract or state regulation)
