What You'll Do
- Connect patients with healthcare and community resources
- Provide outreach, education, and care coordination support
- Advocate for patients and help reduce barriers to care
- Collaborate with medical providers and care teams
- Document interactions and services in the medical record
Minimum Qualifications
- Community Health Worker (CHW) Certification required
- Strong communication and patient advocacy skills
- Knowledge of community resources and social service programs
- Passion for serving underserved and vulnerable populations
Experience working with homelessness, behavioral health, substance use, housing resources, HIV services, or telehealth outreach is highly valued.
