Community Health Worker
The Community Health Worker (CHW) supports patients in addressing social and health-related needs by connecting them to appropriate services and resources. This role serves as a liaison between patients, medical teams, and community partners to help ensure smooth care coordination before and after hospital discharge.
Responsibilities include patient screening and enrollment, transportation support, patient education, follow-up, reinforcement of treatment plans, and appointment coordination. The CHW works closely with the interdisciplinary care team to support patient recovery, self-management, and overall health outcomes.
This position requires in-person patient visits and post-discharge telephone follow-up. Strong communication, critical thinking, customer service, and independent work skills are essential.
Experience/Requirements
- High school diploma or equivalent required
- Must be able to speak, read, and write English
- Experience in home health, community case management, or public health preferred
- Knowledge of community resources, entitlements, and third-party reimbursement preferred
- Bilingual skills preferred
Knowledge, Skills, and Abilities
- Knowledge of chronic health conditions and behavioral health
- Strong customer service, organizational, and interpersonal skills
- Ability to use motivational interviewing techniques
- Effective communication with patients, families, and healthcare teams
- Ability to work collaboratively and independently
- Proficiency in documentation, telephone, computer, and Microsoft Office
- Ability to work flexible hours and move throughout the hospital as needed
Salary Range: Minimum 20.52/hour - Maximum 33.88/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
