Community Health Program Manager

The position is responsible for the design, development, implementation and evaluation of one or more health improvement and community health programs. The individual will engage with community leaders and internal stakeholders to develop and implement policy, systems, and environmental approaches to addressing health needs, particularly as they relate to tobacco, healthy eating, and community health. This role is intended to operate within established strategy and guidance.

Implement evidence-based approaches for community health improvement initiatives and programs with the goal of improving the health of BCBSMN members, Minnesotans, and their communities.

Coordinate cross-functional planning and takes plans from conceptualization through implementation. Develop and continually update project plans, timelines, and deliverables while coordinating multiple sub-processes to expand and/or improve projects.

Manages vendor contracts and project budgets.

Develop partnerships and collaborate with partners across sectors. This includes building and maintaining relationships with key external stakeholders such as public health agencies, community-based organizations and coalitions, community leaders, and State agency staff, and representing Blue Cross priorities in these settings.

Leveraging community health insights and impacting key business priorities and consulting on opportunities to advance community and member health.

Implementing an evaluation and communications strategy to demonstrate and communicate impact

Managing an adaptable work schedule that requires in-state travel and flexible work hours to respond to community partners' needs and opportunities.

Assessing the need and negotiating the provision of technical assistance to internal stakeholders and community partners that advances their contracted work.

3+ years of related community health/corporate affairs professional experience.

Bachelor's degree; in lieu of a degree, an additional two years of relevant experience beyond the qualifications listed above may be accepted.

In-state travel is required.

Hybrid role. Teleworking is working full time remote. Hybrid is a minimum of 2 days onsite. Onsite is full-time onsite.

$79,100.00 - $104,800.00 - $130,500.00 Annual

Pay is based on several factors which vary based on position, including skills, ability, and knowledge the selected individual is bringing to the specific job.
