Community Health Partner

To support members with complex health needs, the full-time remote Community Health Partner will manage a caseload, collaborate with RN Care Managers, and develop care plans addressing social determinants of health. Key Responsibilities Collaborate with RN Care Managers to support members with complex medical and behavioral health needs Manage a caseload of members, identifying and addressing social, behavioral, and environmental barriers to optimal health Conduct assessments and refer members to appropriate behavioral health programs as needed Required Qualifications 2+ years of experience in care coordination, community health, health promotion, or a related field Strong critical thinking and problem-solving skills Experience with documentation and effective communication Demonstrated ability to build rapport with members and navigate emotional barriers Proven track record of collaboration and team effectiveness
