Community Care Liaison

Royal Home Care Services is a non-medical home care agency serving clients in Illinois. We provide supportive home care services that help individuals remain safe, comfortable, and independent in their homes. We are seeking a professional and motivated Community Care Liaison to build referral relationships, promote our services, and support client growth in the Oak Forest and surrounding service areas.

Responsibilities:

  • Build and maintain relationships with referral sources, including hospitals, case managers, senior communities, physicians' offices, and community organizations.
  • Promote Royal Home Care Services through outreach, networking, and community events.
  • Educate clients, families, and referral partners about our home care services.
  • Follow up on referrals and coordinate with the office team for intake.
  • Track outreach activities, referral sources, and follow-up status.
  • Assist with basic marketing materials, community presentations, and agency visibility.
  • Maintain professionalism, confidentiality, and compliance with agency policies.

Qualifications:

  • Experience in home care, healthcare marketing, community outreach, sales, or business development preferred.
  • Strong communication and relationship-building skills.
  • Professional, organized, and comfortable speaking with referral partners and families.
  • Knowledge of the local healthcare or senior care community is a plus.
  • Basic computer skills, including email, Microsoft Office, or similar tools.
  • Valid driver's license, reliable transportation, and ability to travel locally.
