nnChestnut Health Systems is seeking a compassionate, organized, and community-focused Community Access Navigator to support the Just Care for Families program serving Klamath County.
nnIn this role, you'll help strengthen families by connecting them with essential resources, building partnerships throughout the community, and managing the Care Store, an innovative program that helps families access clothing, household items, family activities, and other basic needs.

Responsibilities
n- Manage the day-to-day operations of the Just Care for Families Care Store, including inventory, organization, and accessibility.
- Coordinate community donations of clothing, household items, family activities, and other essential resources.
- Build and maintain partnerships with community organizations, businesses, and service providers.
- Identify and connect families with food resources, community events, transportation, and other supportive services.
- Provide information regarding program eligibility and available services to referral sources and families.
- Travel throughout Klamath County to strengthen community partnerships and expand available resources.
- Support development of program workflows and administrative processes.
- Participate in case review meetings and collaborate with the interdisciplinary care team.
- Maintain accurate records while protecting client confidentiality.
- Promote Chestnut's culture of customer service excellence and recovery-oriented care.
- Perform other duties as assigned. n
Qualifications
nEducation
nnCandidates may qualify through either of the following pathways:
n- High school diploma or equivalent with a demonstrated interest in serving individuals and families experiencing housing instability, poverty, or other life challenges.
- Associate's degree or higher with experience working with underserved populations preferred. n
Skills & Knowledge
n- Excellent communication, organization, and time management skills.
- Strong relationship-building and networking abilities.
- Creative problem-solving skills and resourcefulness.
- Basic computer proficiency, including Microsoft Outlook, Word, and Excel.
- Ability to work independently while collaborating with an interdisciplinary team.
- Passion for serving diverse communities with compassion and respect. n
Additional Requirements
n- Ability to travel throughout Klamath County, including up to approximately 300 miles per week.
- Ability to work both in the office and throughout the community.
- Occasional evening or weekend work as needed.
- Valid driver's license and ability to meet agency driving requirements. n
