To support a growing practice, the full-time Colorado Licensed Case Management Coordinator will manage relationships with county and independent case managers, ensuring seamless service authorizations and care plan updates in a remote setting. Key responsibilities Lead coordination with county and independent case managers for service authorizations and care plan updates Build and maintain relationships with Colorado Case Management Agencies (CMAs) for case reviews and resolutions Support authorization operations for Colorado's HCBS waiver programs, ensuring compliance with policies and requirements Required qualifications 5+ years of case management experience in Colorado Medicaid LTHH settings Proven ability to collaborate effectively with county and CMA case managers In-depth knowledge of Colorado's LTHH ecosystem, including HCPF policies and prior authorization operations Experience working with patient families and clinical teams for care continuity Bachelor's degree in Healthcare Administration, Business, or equivalent preferred; Colorado Medicaid certification strongly preferred
