nOverview
nIn this clinical collaborative physician role, under supervision of the CMO, you will have several clinical responsibilities, along with some specified administrative responsibilities. While providing day-to-day clinical care to residents residing in facilities as well as home bound patients, you will practice and role-m odel ethical and evidence-based decision-making to promote the vision and mission of the company to other practitioners and staff.
nResponsibilities
nProvides high-quality complex and geriatric care to residents and patients across settings in partnership with non-physician practitioner and other interprofessional team members to an identified panel of patients across various assisted living and skilled nursing facilities
nKeeps high engagement with facility leadership teams, nurse practitioners/physician assistants, patients, and families
nProvides collaborating physician duties for on average of 5 but up to 15 nurse practitioners and physician assistants (based on state regulatory parameters) to support them in their clinical roles
nBased on state and regional regulatory frameworks, audits up to 10% of practitioner medical orders and/ or documentation for each provider monthly
nIf needed, assumes medical directorships for skilled and/or other senior living facilities
nHelps identify new growth opportunities with skilled facilities and/or other senior living facilities in the market/region
nParticipates in company frontline /backup call processes for timely response to acute changes of condition to prevent avoidable hospital transfers, or to meet other clinical needs
nKeeps a high focus on quality outcomes including but not limited to care path compliance and productivity, prevention of avoidable transfers and decrease in polypharmacy and costs of care
nAttends required trainings, and meetings as mandated by the CMO or other leaders to assure best care delivery by the company
nQualifications
nEducation/Experience:
nGraduate from an accredited school of medicine and is a Doctor of Medicine
nTwo or more years of administrative leadership experience in healthcare
nClinical medical experience of seven years or more preferred
nCertifications/License/Other Special Requirements
nBoard certification in Family Medicine, Internal Medicine or Geriatrics; qualification in Geriatrics preferred
nPracticing physician currently licensed under the provisions of the State Board of Medicine
nActive unrestricted licensure to practice in state of employment
nCurrent Drug Enforcement Administration registration
nQualifications:
nAbility to communicate effectively with patients and other healthcare support staff
nAbility to treat patients and their families with compassion and understanding
nAbility to work for long periods of time with patients who need special attention
nAbility to display patience with c hildren and adult patients who fear medical treatment
nExcellent computer and electronic record-keeping skills
nExcellent organizational and management skills
nExcellent decision-making, problem-solving and cognitive abilities
