Collaborative Care CoordinatorFull-time Rosenberg, TXSalary$21.69 to $25.25/hour based on years of experienceThe Collaborative Care Coordinator (CCC) provides direct, coordinated services to individuals with co-occurring intellectual and developmental disabilities (IDD) and mental illness (MI) who are enrolled in the Texana Outpatient Biopsychosocial Intervention (OBI) program. Utilizing a collaborative, biopsychosocial approach, the CCC delivers comprehensive case management and skills training to individuals and their families or support systems to promote recovery, stability, and prevent crisis situations. The CCC manages a caseload of up to 15 individuals and works under the supervision of the IDD Intervention Services Team Leader.
This position includes participation in a rotating after-hours, on-call crisis coverage, providing crisis intervention and resolution via telephone or audio-visual platforms as needed. The CCC is responsible for the following:Assessing initial levels of functioning using standardized assessment tools and monitoring progress to evaluate overall improvement.Conducting comprehensive biopsychosocial assessments to identify areas of need, prioritize treatment goals, and determine appropriate supports.Developing and maintaining a Person-Centered Recovery Plan (PCRP) that guides treatment goals, skills development, and addresses barriers to success.Coordinating referrals, tracking follow-up, and consulting with mental health providers, specialized therapy clinicians, and medical service providers.Providing mental health wellness support and skills training aligned with foundational needs identified in the PCRP.Assisting with individuals with medication management and facilitating access to mental health and specialty services.Ensuring adherence to, and promotion of, Texana's Compliance Policies and procedures.Travel throughout a 6-county service regionMaintaining a standard work schedule of Monday – Friday, 8:00am – 5:00pm, with flexibility to accommodate client needs.Required ExperienceMeet the requirements as a QIDP and a QMHP: 1 year experience working with persons dually diagnosed with IDD and a mental illness2 years of experience working with individuals with IDD who have mental health and behavior support needs or linking people with IDD to mental health supports1 year of experience working directly with individuals with IDD or other developmental disabilitiesPreferred ExperienceExperience in crisis prevention and managementRequired Education, Skills and/or CertificationsBachelor's or advanced degree from an accredited college or University with a major in social, behavioral, or human service field including but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education and criminal justice.Texas Driver License in good standing as defined by Texana Center's procedure.Preferred Education, Skills and/or CertificationsSpanish speakingJob related Physical RequirementsMust be able to perform SAMA and CPR.
