Seeking an experienced and compassionate Cognitive Nurse Practitioner or Physician Assistant, the full-time remote provider will deliver comprehensive telehealth care to patients with Alzheimer's disease and other dementias, collaborating with a multidisciplinary team to manage cognitive disorders and support patient education and care planning. Key responsibilities Conduct comprehensive telehealth visits for patients with cognitive and memory disorders Collaborate with interdisciplinary team members to create and implement individualized dementia care plans Provide education and support to patients and caregivers to enhance care quality and reduce distress Required qualifications Active, unrestricted NP or PA license and completion of an accredited Nurse Practitioner or Physician Assistant program Minimum of 2 years of clinical experience as an NP or PA within geriatrics or neurology Strong preference for direct experience in dementia and cognitive care Proficiency with telehealth technologies and EHR documentation Commitment to continuous learning and evidence-based practice in dementia care
