  • Full Time Days - home visits - Lakeshore & Grand Rapids
Atrio Help at Home:
  • All shifts- Flexible Scheduling - Home Visits - Lakeshore & Grand Rapids
Care Resources PACE:
  • Full Time Days - home visits
Faith Hospice:
  • Full time 1st shift - home visits
  • Full time 1st shift - facility
  • Part time 1st shift every other weekend - facility
  • Part time 3rd shift 2 days per week - facility

Responsibilities
  • Provide direct patient care to residents in community or home settings, including basic life support, hygiene, mobility assistance, and daily living activities.
  • Monitor patient condition, report changes, and document in EHR systems accurately.
  • Assist with feeding, medication reminders, and coordination with nursing staff and supervisors.
  • Ensure safety through proper body mechanics, fall prevention, and adherence to care plans.
  • Support geriatric and memory care needs, including Alzheimer's and long-term care considerations.
Qualifications
  • CNA with certification and current license
  • Basic life support, patient care, and manual handling skills
  • Experience in home/community care and geriatric settings
  • Proficiency with EHR systems
  • Knowledge of physiology and nursing home care; ability to support memory care and senior patients
  • Strong patient service orientation and ability to work in long-term or post-acute care environments
