Working remotely, the full-time Licensed Clinical Care Specialist will assess Medicare, Medicaid, and Group Account members, create actionable care plans, and coordinate care for chronic conditions while promoting health goals through a Whole Person Care approach. Key responsibilities Telephonically assess members and develop measurable care plans to guide progress toward health goals Use nursing judgment to manage acute situations and facilitate safe transitions of care Collaborate with a multi-disciplinary team to address members' physical, environmental, and psycho-social health issues Required qualifications BSN or MSN degree in nursing or equivalent Minimum of 3 years of clinical nursing experience with a focus on adult chronic conditions care management Active Compact nursing license and individual licenses in non-compact states Fluency in Spanish and English Experience with motivational interviewing and knowledge of MCG or CMS guidelines
